* The annual maximum benefit increases when you meet the Preventive Care Incentive criteria.

Preventive care includes annual teeth cleaning, a thorough mouth exam, and a schedule of routine X-rays.

These services are covered at 100% when you use an in-network provider. Best of all, when you follow the recommended dental preventive care schedule, your annual maximum benefit increases by $250 for the following year. But, if you skip preventive care one year, your maximum resets to the lowest plan level the next year.

In summary:

  • Your maximum boost is $500 by year three.
  • This applies to basic and major care only.
  • Orthodontia has a separate lifetime maximum.

Before you have any major dental work, ask for a predetermination of benefits from your plan.

This helps you and your dentist understand what is and is not covered and what you might owe after your benefits have been applied. This won’t interfere with your care, it just helps you plan financially. You may be responsible for some costs.

If there’s more than one way to treat your condition, Aetna will cover a less costly treatment if it’s widely accepted in the dental profession and it meets national dental care standards. You’ll be responsible for any cost difference if you choose a more expensive treatment option. Check the Dental Summary Plan Description for full details.
